Prior to your first visit, please either submit our ONLINE NEW PATIENT FORM, or download the PDF file HERE. You can also arrive at our office 15 minutes early for your appointment to complete it.

Occasionally, last minute emergency patients can delay our schedule, so please allow extra time for your appointment. We value your time and will try to keep you updated when delays occur.

Parking:
In rear of both the Barlow Building and the adjacent Ritz Carlton Office Building off of The Hills Plaza. Additional parking is available across Wisconsin Avenue at “The Collection at Chevy Chase Center.” Please allow extra time for parking, especially during inclement weather.
Handicapped:
The Barlow Building is wheel chair accessible from Wisconsin Avenue and from the parking garage at the rear of the building. Our office is specially designed to accommodate all handicapped patients.
